What is a Summer Camp medical certificate of good health?
A medical certificate of good health confirms that you do not suffer from any serious physical or mental health conditions, and are fit and well enough to travel and work in the USA. This medical assessment is required as part of the application process for an American Summer Camp experience when applying to work as camp staff or counsellors.
ZoomDoc medical certificates are accepted by all American summer camps such as Camp America and AmeriCamp and we will complete your specific camp medical form for you for free as part of the process.
What information and medical evidence do you require from me?
After purchase, you will receive a unique certificate booking reference number, which will allow you to upload your medical evidence via a simple form.
To allow us to assess your condition and issue a medical certificate, we will require the following information:
- A photo of your passport or driving license
- Your completed online medical form
- Your Summary Care Record *
- A 30 second video upload to confirm your identity and that you have no infectious diseases
- Details of your regular NHS GP (if you have one)
- The medical form provided by your camp
* Your Summary Care Record is a digital health summary with your NHS GP that shows your current health conditions and any significant past medical health conditions, medications and immunisations. You can access this information via the NHS app, or you can ask your GP surgery to send you a digital or printed version.
Who signs the certificate and how do I know it’s legitimate?
Once your medical evidence has been checked your certificate will be authorised and signed by one of our selected Doctors.
All our doctors are UK GPs registered with the GMC (General Medical Council) and the FCDO (Foreign Commonwealth and Development Office).
Zoomdoc Health is an authorised medical provider regulated by the Care Quality Commission. Our contact details will be shown on your certificate with a QR code enabling a verification check of your certificate.
How long is this medical certificate valid?
Our Summer Camp medical certificates are valid for 180 days (6 months) from the date of issue. The separate American camp medical form, which is also completed by our doctors, is typically valid for up to 12 months.
What happens if you cannot validate my medical evidence?
Our doctors thoroughly review your medical evidence and may call you if they have any questions.
If for any reason, we are unable to validate your evidence or are unable to supply you with you a certificate you will be notified and given a full refund.
Will you complete my camp medical form?
Yes. After the assessment your ZoomDoc Doctor will complete your camp specific medical form at no extra cost.
