If your request falls outside our standard certificates, one of our GMC-registered UK doctors may be able to provide an individual medical opinion following a clinical assessment.

Each request is reviewed on its own merits. If clinically appropriate, your doctor can issue a personalised medical letter addressed to a named organisation or simply “To Whom It May Concern.”

Our doctors provide independent medical opinions based on:

  • Your completed medical questionnaire.
  • Supporting medical documents or evidence you upload.
  • A telephone consultation where further clarification is required.

Every request is assessed individually. A medical letter will only be issued where the doctor is satisfied it is clinically appropriate and supported by the available information.

What will I receive?

You will receive a verifiable digital PDF letter signed by a medical professional and sent directly to your mobile, containing the following details:

  • A personalised medical letter written by a GMC-registered UK doctor.
  • An individual clinical assessment based on your questionnaire and any supporting evidence.

* ZoomDoc Health is a private UK doctor service – the medical certificates we provide are not the same as NHS Med3 Fit Notes, which are only available from healthcare professionals delivering NHS services.

 

As an online service, we are unable to provide GP sickness notes for the UK Home Office, immigration or asylum appointments, court appearances, jury service or workplace tribunals, legal reports, sports medicals, extreme sports, any driving based roles or fitness to fly after a lower limb fracture, recent heart attack or stroke.

  • A photo of your passport, driving license or workplace, university or college ID
  • Completion of an online medical questionnaire (post purchase)
  • Upload a 30-second to 1-minute video describing your symptoms
  • Details of your regular GP (if you have one)

What is a Bespoke Medical Letter?

A bespoke medical letter is a personalised document prepared by a doctor when your circumstances are not covered by one of our standard certificates. Rather than completing a pre-written template, your doctor writes a professional medical opinion that reflects your individual situation and the clinical information available at the time of assessment.

Depending on the nature of your request, your doctor may confirm a diagnosed medical condition, explain how a medical condition affects your daily activities, comment on a period of illness or recovery, or provide an opinion regarding your fitness or temporary unfitness for a particular activity. Every letter is written honestly, accurately and in accordance with UK medical practice and GMC guidance.

Who Might Need a Bespoke Medical Letter?

Patients request bespoke medical letters for a wide variety of reasons. Some need confirmation that they are medically fit to return to university after a period of illness, while others require supporting evidence for their employer following time away from work. We also receive requests from individuals who need medical confirmation for workplace adjustments, educational institutions, travel providers or organisations requesting clarification about an existing medical condition.

In some cases, a doctor may also be able to provide a medical opinion regarding requests for exemptions, such as specific workplace footwear requirements, shaving exemptions or the ability to wear a face covering where a genuine medical condition significantly affects this. Every request is considered individually and approval cannot be guaranteed simply because a request has been submitted.

If your circumstances are unique and are not covered by one of our existing medical certificate services, a bespoke medical letter may be the most appropriate option.

How Our Doctors Assess Your Request

Every bespoke medical letter begins with a detailed clinical assessment. You will be asked to complete a medical questionnaire describing your circumstances and, where relevant, upload supporting medical documents such as hospital letters, clinic correspondence or previous investigation results.

Your doctor will carefully review all available information before deciding whether they are able to provide the requested medical opinion. Occasionally, further clarification is required and one of our doctors may contact you by telephone before reaching a decision. This ensures that every letter issued is based on sufficient clinical information and reflects an independent professional opinion.

Our doctors will only issue documentation where they are satisfied that it is clinically appropriate and supported by the available evidence.

Situations Where We Cannot Provide a Bespoke Medical Letter

Although our doctors can assist with a wide range of requests, there are important circumstances where it would not be appropriate or safe to provide documentation through an online assessment.

We are unable to prepare reports intended for courts, tribunals, expert witness proceedings, Home Office applications or immigration matters. These requests usually require specialist medico-legal reports, access to full medical records and, in many cases, an in-person assessment.

Similarly, we cannot provide medical certificates for activities that require a comprehensive physical examination or additional investigations before fitness can be determined. This includes scuba diving medicals, skydiving medicals and certificates for competitive sporting events such as marathons, triathlons or endurance competitions. These assessments frequently require an ECG, exercise testing or examination by a doctor with specific expertise.

Important Information About Fitness to Fly

ZoomDoc Health provides Fit to Fly certificates where it is clinically appropriate to do so. However, there are certain situations where a remote assessment cannot safely determine whether travel is appropriate.

For example, we are unable to provide a Fit to Fly certificate following a recent lower limb fracture because of the increased risk of venous thromboembolism (blood clots), the potential need for anticoagulation and the requirement for ongoing clinical monitoring. Similarly, patients who have recently experienced a heart attack, stroke or transient ischaemic attack require assessment by their treating clinician before travel can be considered.

Where a patient has significant red flag symptoms or an unstable medical condition, it would not be appropriate to provide remote travel clearance. In these circumstances, we recommend assessment by the treating hospital team or another healthcare professional who can perform a face-to-face examination.

What We Cannot Provide

For patient safety and medico-legal reasons, our doctors cannot provide every type of medical letter.

Medicals Requiring Face-to-Face Examination

These require physical examination and often additional investigations such as ECGs or specialist assessment:

Scuba diving medicals.

Skydiving medicals.

Marathon medical certificates.

Triathlon medical certificates.

Competitive sports medicals.

Endurance event medical certificates.

Sports participation medical certificates requiring ECG or exercise testing.

Fitness to Fly Restrictions

Although ZoomDoc Health provides Fit to Fly certificates where clinically appropriate, there are circumstances where remote assessment is not considered safe.

Our doctors cannot issue a Fit to Fly or travel clearance certificate where there has recently been:

A lower limb fracture associated with an increased risk of venous thromboembolism (blood clots), anticoagulation requirements or ongoing specialist management.

A heart attack (myocardial infarction).

A stroke (CVA) or transient ischaemic attack (TIA).

Significant red flag symptoms.

An unstable or deteriorating medical condition.

These situations require assessment by the treating hospital team or another clinician who can perform an appropriate face-to-face examination before travel.

Will I need to speak to a doctor?

Not always. Many requests can be assessed using your questionnaire and supporting evidence. However, your doctor may telephone you if further information is required.

Can you write a letter to my employer?

Yes, where clinically appropriate.

Can you write a letter for my university or school?

Yes. Our doctors frequently provide supporting medical letters for education providers.

Can you provide a general “To Whom It May Concern” medical letter?

Yes. This is one of the most common reasons patients request this service.

Can you provide legal or immigration reports?

No. We do not provide reports for courts, tribunals, Home Office applications or immigration matters through this service.

    Our medical letters and certificates have QR codes that can be scanned to verify the document's authenticity.

    Once scanned, your personal online results portal is loaded, from where your medical letter can be reviewed and personal details confirmed.

  • Do you provide refunds?

    We only provide full refunds if we are unable to provide a medical certificate.

    Once our doctors have provided a medical certificate, we are unable to process a refund.

  • When will I receive my medical letter?

    Our team will review your evidence and provide you with your medical letter on the same day that evidence is uploaded (between 9am and 9pm GMT, 7 days a week).

  • Who signs this Doctors Letter?
