Why do I need a vaccine exemption certificate?

Vaccine exemption certificates are intended for individuals who cannot receive vaccines for a number of medical reasons, but may still require access to services; locations, or even jobs that require proof of vaccination.

This type of certificate aims to stop vaccine exempt people from being discriminated against or excluded from a number of activities that otherwise require a full vaccination course.

  • A photo of your passport or driving license
  • Your digital Summary Care Record or a doctor’s letter confirming the medical reason for exemption
  • Your completed online medical form
  • Details of your regular doctor (if you have one)
