What will I need to supply?
To allow our doctors to assess your health and issue your medical certificate, we will need the following:
- A photo of your passport, driving licence or workplace ID
- Your completed online medical questionnaire
- A recent medical / health summary
- A short 30โ60 second video describing any recent illness and any current health concerns
- The name of your workplace and the type of work you will be doing
- Details of your regular doctor (if you have one)

Our online service for issuing Olympic fit-to-work certificates is available only for workplace roles that are considered low-risk.
If your role carries a higher level of physical risk, you will require an in-person medical assessment โ so that a clinician can examine your heart and lungs, check your blood pressure, and arrange any necessary eye or vision tests.
